·         Annual Physical, Annual PPD and Lab Reports

You must provide to the staffing manager your current Professional or Non-Professional Skills License or Nurse Aide Certification and the expiration date.  Also, make sure your have provided all information to the staffing office either by E-mail or via fax transmittal.
